HOW TO FILL OUT THIS FORM Please refer to: • The HVAC Rebate Catalog for measure requirements and information. • Complete the table corresponding to the measure in the catalog. • Attach this form to a completed Rebate Application and submit together. GENERAL REQUIREMENTS • Rebates are for upgrades and retrofits of existing equipment, unless specified as eligible for new construction. • Redundant or backup equipment do not qualify. • Only natural gas equipment or equipment additions is eligible for heating system rebates. Equipment fueled by, or added to, electric, propane, oil, or wood are NOT eligible for heating system rebates, unless otherwise noted in measure description. • Measures in this catalog are for standard space heating and cooling applications ONLY. Data centers, ice rinks, and refrigerated warehouses may qualify for custom incentives. The Process Catalog to be used for industrial process related upgrades. • Equipment required by state code are not eligible for rebates. • Custom incentives may be available for measures that do not meet offering requirements.
